How to Schedule Your Clinic

Follow these 6 steps to ensure a seamless vaccine experience for your community.

Access Portal

Navigate to the "Vaccine Concierge" portal on our dashboard.

Print Consents

Print the Flu and Pneumonia Consent form for each resident.

COVID Forms

Separate COVID consents must be completed if requested.

Build Packets

Include: Consent Form, RX (specified pneumonia type), Face Sheet with allergies, and insurance card copies.

Submit Request

Click "Request Vaccine Clinic" below once your packets are ready.

QA Contact

Our QA team will contact you with specific instructions and your clinic date/time.
